Booking forms
A Veterinary referral form and a client contact form are required prior to booking an initial appointment. Existing clients can use our Acuropet App to view available appointments and/or you can request a time using the appointment request form below. We will contact you to confirm the booking.
Veterinary practices or individuals who like to book a presentation or consultation can complete the appointment request form. We will contact you to determine logistics.
Veterinary Referral Form
All clients of Flourishing Qi must have a primary conventional veterinarian. We wish to work in an integrated fashion so the patient receives optimum care.
